PURPOSE

Plans, organizes and coordinates discharge planning activities into the patient’s total care.  Assists nursing services in planning and coordinating the educational aspects of services in the hospital.  Assists in providing for the social, emotional, and psychological needs of patients. Will plan, organize and coordinate admissions and discharge activities for the hospital-based, inpatient medical service for withdrawal from alcohol, opioids, and other drugs, including prescription medications. The case manager will work with the acute hospital staff and providers to organized cohesive care for these patients.

PRIMARY RESPONSIBILITIES

  • Participates in the admission process by explaining admission, continued stay criteria, and accesses patient / family resources for their financial responsibility.
  • Assures that appropriate Medicare notes are reviewed and signed.
  • Participates in development of the patient’s plan of care and completes needed follow-up
  • Reviews medical records, interviews patient and families, and collaborates with the provider, nursing, and other health care team members to assess the social, psychological, and environmental status to coordinate discharge needs and services.
  • Coordinates and participates in multidisciplinary discharge planning conferences.
  • Assists in arranging transportation for patients.
  • Reviews and responds to any complaints or grievances pertaining to the department.
  • Assesses and documents patients living situation, mental status, activities of daily living, ability to function independently, and availability of support system.
  • Coordinates the referral of patients and families to appropriate community based resources in Home Health and Hospice, Nursing Home, Assisted Living, Area Agency on Dying, and DME supplies.  Communicates with these entities to assure a safe transition and continuity of care.
  • Demonstrates an awareness of patient rights and explains to patients and families as necessary.
  • Assists patients with advance directives and work with nursing and families to assure that they are placed in the patient’s record.
  • Maintains a current resource manual.
  • Develops a department budget.
  • Participates in the hospital CQI program.  Performs QI and PI activities within the department.
  • Serves as Intake Coordinator for Swing Bed referrals.  Screens patients for appropriations for the Swing Bed program at Gothenburg Health.
  • Maintains confidentiality of patients, department, and the facility; including compliance with Health Insurance Portability and Accountability Act (HIPAA) regulations.
  • Develops and follows policies and procedures appropriate to the department, as well as Hospital wide policies and procedures.
  • Builds and maintains relationships with other entities for referrals
  • Screen and schedule admissions following Milliman’s admission criteria
  • Provides community education and awareness and onsite staff education.
  • Works effectively as a team member within the facility to provide quality service to other hospital departments through communication, cooperation, and collaboration.
  • Performs post discharge follow up activities including phone and written surveys.
  • Expands job related knowledge and skills for self improvement and adjustment to change.
  • Completes required training and education including department specific requirements.
  • Documentation of Discharge Planning / Case Management activities in the patient’s record.
  • Performs other related duties as needed to support the achievement of departmental goals and objectives.
  • Performs any other duties as may be assigned.

PHYSICAL DEMANDS

  • Work requires sitting, talking and listening to a significant degree and ability to communicate via telephone and person to person. Requires auditory ability sufficient to hear telephone and typical hearing impaired enhancements and to hear normal conversation in an office setting.
  • Requires ability to lift or move up to 10 pounds.
  • Must be able to handle multiple interruptions with patience and tact.
  • Requires vision sufficient to read fine print on documents and reports.
  • Required ability to transfer self to various areas in the facility.

SPECIAL DEMANDS

  • May be asked to attend special meetings and workshops, which may require out of town travel.
  • Patience, tact and cooperation in dealing with doctors, residents, visitors and hospital personnel.
  • Interpersonal skills necessary to gather and exchange information with the provider, patient, family, and others involved in the patients care.

 
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • RN with current license, Social Worker (BSW, MSW or LCSW) or other appropriate experience.
  • Current license/certification in the State of Nebraska.
  • Excellent oral and written communication and interpersonal skills.
  • Demonstrated ability to assess patient’s needs, develop and implement a plan of care and coordinate the discharge planning process.
  • Knowledge of hospital policies, accreditation standards, and compliance requirements.
  • Demonstrated planning and implementation skills.
  • Demonstrated computer skills.
  • Ability to speak and write the English language in an acceptable manner.
  • Maintain knowledge of necessary Social Work training, practices, and procedures.
