PURPOSE

The Behavioral Health Therapist will support the primary care providers and team at Gothenburg Health by providing psychotherapy services to children, adolescents and adults with an identified mental health or substance abuse diagnosis. Services are provided with client-centered and family-centered approaches and are delivered through comprehensive diagnostic mental health assessments, interventions, education, referrals and consultations for patients in the primary care setting.  The Behavioral Health Therapist will share clinic space and resources and will strive to intervene with all patients on the day they are referred. PRIMARY RESPONSIBILITIES

  • Provide brief behavioral and cognitive behavioral interventions for patients
  • Provide medical providers with same-day verbal feedback on client encounters
  • Screen, identify, and manage patients who present difficulties with mental illness, substance abuse, and other social determinants of health risk factors that impact patient’s behavioral health
  • Provide psycho-education for patients during individual and group visits.
  • Provides a range of services including screening for common conditions, assessments, and interventions related to chronic disease management programs
  • Provide crisis triage and intervention as needed
  • Complete various assessments and reports, as appropriate
  • Formulate and administer treatment plans
  • Communicate and collaborate with internal and external individuals and groups to provide excellent care, customer service and cooperation
  • Participate in quality assurance initiatives

PHYSICAL DEMANDS

  • May require sitting and keyboarding/typing for long periods of time.

  • Occasionally lift up to 10-20 pounds.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Master’s Degree
  • Licensed Clinical Social Worker (LCSW) preferred but would consider Licensed Clinical Professional Counselor (LCPC) 
  • Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by administration.
  • Maintain confidentiality of patient medical record, including health and financial, clinic and hospital
  • Respond to stress and emergency situations with ability to exercise sound judgment skills.
  • Ability to meet physical requirements specified above.
