How can I obtain copies of my medical records?

Gothenburg takes great care to ensure your health information is kept private and secure.  If you need copies of your medical records for yourself or a third party, we can help.

To request your records from Gothenburg Health (including radiology reports and images) download, fill out, and sign the form below.  Be sure to include the dates of service.  Without that information, the request cannot be completed.

What are the clinic hours?

The Clinic at Gothenburg Health is open from Monday-Saturday each week with some walk-in availability. Call 308-537-4066 to schedule your appointment. 

Monday - Friday: 7 a.m. to 5 p.m.
**Walk-in availability from 7 a.m.-8 a.m.
Saturday: 8:30 a.m. to 11:30 a.m.
**Walk-ins allowed

How can I pay my bill?

Contact the Business Office at 308-537-3661, option 2. You will reach a Patient Accounting Representative, who will be able to assist. Or, use the convenient EPAY button at the top of this web page.
