Patient Care Coordinator (Full-Time)

Position Summary

Reporting to the CNO, the patient care coordinator supports patients and clinic staff in the departing and referral processes for all clinic patients and will support various hospital departments with incoming orders, scheduling and prior authorization. This is a highly visible position that interacts with patients and their families along with hospital staff both internally and at all referral facilities to coordinate access to needed services/specialists. The patient care coordinator must effectively deal with challenges and conflicts working with the patient, family and/or caregivers, and the multidisciplinary care team. 

Accountabilities

  • Schedule any laboratory, radiology and treatment needs stemming from the provider visit and departing the patient when the visit is complete with any needed follow-up appointments.

  • Begins any needed prior authorization for additional care ordered by the provider and schedules the patient for the needed services once the authorization is complete.

  • Works with outside offices/hospitals to obtain timely appointments for services needed and not offered at Gothenburg Health while gathering and sending any needed information to the accepting facility.

  • Updates patient information as obtained and completes documentation according to identified process and workflows for consistent communication and care.

  • Works with outside facilities and providers to schedule patients for services at our facility making sure all needed documentation and prior authorization is obtained.

Minimum Qualifications

  • High School diploma or GED
