Foot Care/Population Health Nurse

Position Summary

Reporting to the Director of Population Health, the foot care/population health nurse supports patients in assessments and treatments determined by the provider. They also provide foot and nail care services in the outpatient clinic setting. This is a highly visible position interacting with the public and internal hospital departments. This role must consider spiritual/religious beliefs, caregiver and other relationships, living status, availability of social services and community programs, use of alcohol and other substances, and behavioral health status. Must effectively deal with challenges and conflicts working with the patient, family, and/or other caregivers, and the multidisciplinary team.

Key Accountabilities

  • Complete needed assessment and treatments for foot care independently from a provider by following pre-determined workflows.
  • Complete documentation on patient care or assessments as determined by processes and workflows for holistic care of the patient.
  • Determines eligibility and conduct needs assessments for patients referred to care coordination services while developing individualized plans of care to assist the patient in their healthcare goals.
  • Identifies potential problems and barriers to care and assists in resolving to help patients maintain their health and determine their healthcare goals.
  • Acts as a liaison of the patient with their other providers and services to assist patients in getting their needs met.

Minimum Qualifications

  • LPN or RN Nebraska licensure.
  • BLS required.
  • Valid Nebraska driver’s license.
